This is what you need to do to get your prize.

1.Please send a message to Sempio's email: [email protected]

Please send us your copy of driver's license with correct address to receive your prizes.

*Your facebook name and your document name has to be same. If it's not, please send us a file that can proof yourself.

2. Please fill out W-9 form ( this will be attached to via email and requires to 'Chef Idea Prize' &' food stylist prize')

3. Soon as first two steps are clear, and when documents are verified your prizes will be sent.

* If you are able to pick up the prize, Sempio will ask you to visit office. Just in case of lost or missing of the prize. If you cannot, we will sent out the gift card(prize) for you. Soon as prize sent out to the receiver, it will be notify and we are not responsible of sent out prize.

4.When you receive prize please send us back a form that you have received the prizes.
